Self Paying Clients:
Please note that I do not do same day appointments for new clients. Self pay clients should click here to submit an appointment request and begin the process to get on the schedule. Once we find a time that works for you, I will send out an intake form and my Practice Policies. The intake form allows me to prepare for your session, so please be sure to include all relevant medical history and anything else that you think will be helpful in your care. Please read through the policies and sign the agreements promptly so I can confirm your place on my schedule. Here is my HIPAA policy for you to read. If you would like a printed copy, please let me know by email so I can have one available at your appointment.
Insurance Clients:
As of September 2024, I am accepting a limited number of new insurance clients. I wish I could provide care to everybody that reaches out, but there is only one of me and I can only do so many sessions in a week. If you would like to check availability for insurance based appointments, please email me with your referral information to see if our schedules work together.
I am a credentialed provider with Regence, First Choice and can also bill out of network for some United plans. Please note that this service requires a prescription from a primary care provider and verification of benefits and does not guarantee insurance reimbursement. If your insurance denies reimbursement you will be responsible for payment.
